healthgram - Personal Health Record is a secure web-based application that stores important health for our plan members and clinicians.  Health information is collected automatically and stored from the healthplan and the various wellness and care management programs that the member is part of.  Members can also enter their own information directly into the record.  The information can be accessed and made available to authorized medical professionals and entities, as needed.
